Membership Agreement and Terms
Welcome to Skin Spa Symposium’s membership program! By joining, you are saving money and becoming more vested in your continuing skincare.
Please initial the following terms and conditions in the space to the left of each numbered line.
1. _______You agree to a one- time enrollment fee of $20.00, which is due today.
2. _______You will pay your monthly membership amount of _______________ by a credit/debit card on file on the following day each month__________.

Please advise me of any changes regarding the credit card on file or the need to change the chosen date of your monthly payment.
Membership Details & Notice:
3. _____ Your membership begins on _________________________ and will be automatically renewed at the same day of each month at the same rate unless you give thirty days written notice.
Please email email@example.com to provide your thirty days written note. In the subject-line please type “Cancel Membership”
Cancellation & Scheduling Policy:
4. ________You must provide a minimum 24-hour notice to cancel an appointment. If such notice is provided, you will not be charged a cancellation fee for your appointment. If 24 hours cancellation is not given, you agree to pay a $30 cancellation fee. If you do not show up for your appointment you agree to pay a $30 no-show fee.
5. _________ You will be responsible for any fees Sherry Lesko/Skin Spa Symposium incurs as a result of insufficient funds or a failure to inform me that your credit card has changed or expired.
6. ________ You acknowledge receiving and reading a completed copy of this agreement before signing. By signing below, you agree to the terms and conditions as stated in this document.
Skin Spa Symposium agrees to notify all members of any necessary changes to the membership program.
